To apply for a medical grant from the Sir Dorabji Tata Trust, you are required to:

Visit the Trust Office at Mulla House, 3rd Floor, 51 M.G. Road, Mumbai 400001 between 3:00 pm to 5:00 pm from Monday to Friday, and pick up the application form. |

Please ensure that you have all the following documents: |

Copies of Hospital/Medical Bills |

Proof of Residence i.e. Ration Card or any other document |

Recommendation Letter from Medical Social Worker from a general, municipal or government hospital. |

Employers Reimbursement |

Medical Insurance |

The completed forms should be submitted in person to the Trust Office along with the required documents within a month of collecting the form. |
